 We are excited that you have chosen to enroll your child at St. Paul’s Lutheran Early Childhood Center.  Attached are the preschool forms you will need to fill out and return before your child begins preschool.

 The physical form must be completed yearly as required by the Department of Health and Human Services.   Forms are good for one year from the date on the physical.  Please plan ahead to schedule an appointment.  All children must have a physical form turned in before their first day of preschool.

 Immunization forms MUST also be updated yearly and turned in by the first day of preschool. 

 I have included a checklist of forms to be turned in to assist you.  Some of the forms are double sided.
